Completing the Required Forms
The first phase is filling out the Application for Treatment form. This forms allows Dr. Ferguson to fully understand your problem. We ask that you be as complete as possible. This will aid the doctor in your diagnosis work-up. If an automobile accident or an on the job injury is the cause of your problem, you will be asked to fill out another form. These forms should be turned into the receptionist with your insurance card. During your examination, we will attempt to contact your insurance company for the details of your coverage. We will explain this information to you.
Meet Your Doctor
Next you will be taken to the examination room and meet with one of the doctors. At this time he will go over the forms you completed and will take a thorough history including family history and any details not specified on the Application for Treatment form. If spinal pictures are required, the doctor will explain what views are necessary and why. We offer the finest in radiographic examination and our equipment is state of the art. We have gone to great measures to ensure the safety of our patients and to provide the best services possible. A trained staff member will take your radiographs.
Recommendations
At this time the doctor could recommend some form of physiotherapy. This will aid in the reduction of muscle spasm and swelling. You will then be given instructions for home care of your condition. An appointment will be scheduled for the following day for a doctor to correlate the findings from the x-rays and the examination. Physiotherapy could be applied again if the condition warrants.
Discussion of the Doctor’s Findings
On the next appointment, you will consult with the doctor for a Report of Findings. At this time, you and the doctor will look at your x-rays, as he explains the findings and discusses your examination. If the doctor feels your condition would benefit from chiropractic or acupuncture, you will be accepted as a patient. The doctor will go over your treatment plan, exercises, and home therapy at that time. If applicable, we encourage all of our new patients to bring their spouse for the Report of Findings. This helps the spouse understand the condition and what help she or he can be. If necessary, the doctor will issue an excuse for the spouse to miss work.
